complete process. It is designed to assure that there is a good
understanding and agreement between the potential resident and the
Program Director at Daisye's Place. The process assures -

How are women selected for residency at
Daisye’s Place?
A recommendation from a rehab program director is the required first
step. This is followed by an application and
personal interview with the director.

Who makes the final decision on which
applicants are selected to become residents?
The Program Director and Executive Director of Daisye's Place make
the final decision.

When can applications be completed and
submitted for consideration?
The application process can start up to 60 days prior to graduation
from a rehab facility. Each application is evaluated when it
is received by the Program Director.
Successful completion of a rehab program, a positive recommendation
from the Program Director of the rehab facility, need for
transitional assistance, room availability, desire to learn,
become independent and have a new beginning, and successful
completion of the application process.
Yes, resident rooms are filled a soon as possible.
Applications are considered valid for 60 days form receipt by the
Program Director. Women submitting applications, however, are
encouraged to call weekly and speak with the Program Director to
keep their application on a high-priority basis.

Acceptance to Daisye's Place is a three step process
-
The Application for Admission must
first be completed to get the
process started. At that time, the applicant must also
agree and sign the Residential Agreement, Personal Items
Agreement, General House Rules Agreement, and the Financial
Responsibility Agreement. These four agreements are signed
early in the process in order to assure that there is no
misunderstanding about rules, obligations, etc.

Completion of the personal interview is the second step.

Completion of the Health Screening Form by a licensed physician
is the last step in the process.
Each step must be successfully completed prior to an applicant being
accepted for residency.
